A large number of our elders are part of the Long-Term Care program. This number can vary from month to month, with clients voluntarily accessing services such as socials, church and grocery store outings.
The nursing program in M’Chigeeng supports the efforts of Long Term Care with ongoing communication and the sharing of information regarding local initiatives and helps to coordinate doctor visits and transportation services. The nursing staff also assists some post-operative communications for elders and the CHR has provided translation services for elders who need this service.
The elders have been involved with the Diabetes program by providing a tuck shop where the children can exchange sugary snacks for small items to promote physical activity. The elders run the tuck shop and have positive interactions with the youth in the community.
The Long Term-Care Case Manager and the Nursing Supervisor attendant number of meetings with the local hospitals to improve discharge planning for our community members to ensure that clients being discharged are seen in a timely manner with enough lead time to prepare for them.
Some of our elders that have passed on this year have been supported in their journey through palliative support from our Long-Term Care and nursing program.
The local elder’s facility continues to host events for all the elders to get together for socials, workshops and spiritual services.
The nursing program also provided home visits for flu shots with the elders who were unable to come to the local clinic.
