M'Chigeeng Home Care Program
These programs are provided to people mainly in the home. The belief of care is to help people keep their independence in their own home and allow them to be close to their loved ones as long as possible. The program is designed to support and improve the care provided by the family and community.
Personal Support Workers assist individuals with personal hygiene, light meal preparation, medication reminders and safety or security checks. Additional services may include nursing, meals on wheels and respite services.
Services are provided based on needs identified through an assessment completed by a nurse. Referrals can be made individually, or by others such as family members, friends, family physician, neighbors or hospitals.
Referrals can also be completed to other care providers such as Mental Health, Traditional Medicine, Speech Therapy, Occupational Therapy, Physiotherapy, Foot Care, Homemaking, Diabetes Program, Dietician, Nutritionist and other Health Services Programs through the Health Centre.
